Player Registration & Waiver

This Contract is a binding agreement by the player and/or the player's parent/guardian to have the above-named player play on a Canadian Hockey Club (CHC) team for the 2026 season.

It is understood and accepted that any hockey pants, jerseys, and socks supplied by the team remain the property of the team and will be returned to George Maris when requested, or before August 18, 2026, whichever comes first. Otherwise, a fee of $459.00 will be paid to George Maris to replace these articles.

If for any reason the player quits the team, is suspended from the league, or commits any malicious acts, he will be expelled from the team and will forfeit all money paid and due.

This year's dues are $1,650.00, payable in 5 equal installments of $330.00 due January 15, February 15, March 15, April 15, and May 15, 2026 (a 5% sibling discount is available). A $330.00 deposit is required to secure the player's spot and is non-refundable. The full balance is due in full by May 15, 2026. A $100 service fee will be added to the amount owed if any outstanding balance must be sent to collections, and any additional collection fees will be the responsibility of the player and/or his parents. All payments are payable to the order of George Maris.

Payment Methods

As parent/guardian of the above-named player, I hereby grant permission for him to participate in the activities of the Canadian Hockey Club (CHC). I hereby waive, release, and forever discharge said Canadian Hockey Club (CHC), its officers, members, agents, representatives, George Maris, and coaches from all claims and demands which I, my heirs, executors and administrators, and those of the above-named player have or may have by reason of any personal injury or injuries, property damage, or damage of any nature whatsoever resulting from the participation of the above-named player in any of the activities of the Canadian Hockey Club (CHC) team, its travel, and any consequences arising therefrom. I also authorize Coach Maris to authorize any treatment in the event of injury to my son.

This form must be completed and signed prior to any player participating in any CHC team activity.

By checking the boxes above and clicking “Submit Registration & Generate Signed PDF” below, each signing party agrees that their typed name constitutes their electronic signature, intended to authenticate this document and to have the same legal force and effect as a handwritten, ink signature, consistent with the U.S. Electronic Signatures in Global and National Commerce Act (E-SIGN Act, 15 U.S.C. § 7001 et seq.) and applicable state adoption of the Uniform Electronic Transactions Act (UETA).

Each signing party affirms that they are the person identified in the corresponding signature field (or, for the Player Signature field, the player named above or someone authorized to sign on the player's behalf); that the parent/guardian signing has the legal authority to sign this Contract and Release on behalf of the above-named player if a minor; that they have read and understood the entire Player Registration Contract and Release of Liability set out above; and that they intend to be legally bound by their terms.

Each signing party further consents to conducting this transaction electronically and to receiving an electronic copy of this signed document (the generated PDF) in lieu of a paper copy. This document will be dated and time-stamped at the moment of submission, which constitutes the moment of signature.
